Submitting a Claim

Canada Life manages our medical, prescription drug, dental, personal wellness, and health care spending accounts. To submit a claim online, you can use the GroupNet app. To submit a claim through the mail, visit the Canada Life website to get the form you need. To have your provider – medical, pharmacist, dental office, optical location or paramedical – submit a claim on your behalf provide them with the appropriate policy number.

Use GroupNet to Submit a Claim

To get started using GroupNet:

Go to GroupNet and click register. You will need your plan number (57505) and your member ID (your employee number). Don’t forget to include your banking information so that when you submit claims your payment will go directly into your account.
